The Dietitian Nutritionist, Senior Coordinator works with clients referred to us by our insurance partners who provide Medically Tailored Services. This program type requires the clients to receive a nutrition assessment at least once every six months. The Coordinator will provide nutrition assessments, set goals with the client, and offer the individual any of the other services available through Project Bread. The Coordinator provides ongoing case management support for each client throughout their enrollment in our program. This position will primarily work with members remotely, but the role may include some in-person visits on an as needed basis.
The SUN Bucks Enrollment Assistant will work closely with the Senior Director of SNAP Outreach Programs to help families with school-aged children enroll in SUN Bucks (formerly known as Summer EBT), so they can buy healthy food all summer long. The Intern will be responsible for ensuring applications for the federal SUN Bucks (Summer EBT) program are properly received, entered, and filed, as well as providing over-the-phone assistance to clients, referring them to the FoodSource Hotline when needed. Successful candidates must have strong customer service skills and attention to detail.
